Inform us that you have changed your name

As we need documentary evidence to support your change of name, we are unable to update your details over the telephone.

To enable us to update your details we require a letter enclosing the relevant documentation. Please state your address, along with your policy number(s), to enable us to locate the correct records. If you have also changed your address, please show your new address, as well as your old address, so that this can also be updated.

The table below lists the circumstances in which you may need to change your name, and the documentary evidence that you will need to send us. If you have any questions, they may be covered in the question and answer section at the bottom of this page. Any additional questions can be raised by you in your letter to us.

Reverting to Maiden NameYour original Birth Certificate or A Verification Form from your Solicitor or IFA. Photocopies are not acceptable.

An original or certified copy of a Deed Poll or Statutory Declaration. Photocopies are not acceptable.
Voluntary name change by Deed Poll or Statutory DeclarationAn original or certified copy of a Deed Poll or Statutory Declaration. Photocopies are not acceptable.
Please note: If you have changed your name voluntarily, without Deed Poll or Statutory Declaration, we are unable to change your name on our records.
Parents changing the name of a minorA copy of a Court Order, giving permission for the name change. Photocopies are acceptable.

A copy of a letter from the Court, or a Solicitor, giving permission for the name change. Photocopies are not acceptable.
Please note: If the change of name is due to adoption, we only require written confirmation from the adoption agency. Photocopies are not acceptable.

When we receive your letter any original documents will be scanned on to our computer system and returned to you as soon as possible. If the correct information has been provided, your records will be updated, and we will write to you to confirm this.

Questions and Answers

Why do you require the original documentation (where stated)?
In accordance with The Office of Public Sector Information (previously Her Majesty's Stationary Office) guidance, and to ensure we do not infringe, or cause you to infringe, Crown Copyright we must request original Birth, Death or Marriage Certificates. We will always return your original documentation.

What is a Solicitor or Independent Financial Advisor's (IFA) Verification Form?
This is a document approved by the Association of British Insurers, the Law Society of England and Wales and the Association of Independent Financial Advisers. You can obtain this from a solicitor or IFA to confirm the details from your original birth, marriage or civil partnership certificate. The solicitor or IFA will need to inspect your original certificate when completing the verification form.

Will you return photocopies of documents sent to you?
Yes, we can do this as long as you specifically request this in your letter.
